Sleep-Wake Disorders

What are they?


Your body has an internal "clock" (circadian rhythm) that tells you when to sleep and when to be awake, roughly following a 24-hour cycle. Circadian rhythm sleep-wake disorders occur when your internal clock is out of sync with the schedule your life demands. This leads to difficulty sleeping at the desired time, excessive sleepiness, or both.

The main types include:

  • Delayed Sleep-Wake Phase Disorder: Your natural sleep time is much later than normal (e.g., falling asleep at 2–6 AM and waking at 10 AM–2 PM). Most common in teenagers and young adults.

  • Advanced Sleep-Wake Phase Disorder: Your natural sleep time is much earlier than normal (e.g., falling asleep at 6–8 PM and waking at 2–4 AM). More common in older adults.

  • Shift Work Disorder: Difficulty sleeping and excessive sleepiness caused by working night shifts, early morning shifts, or rotating shifts.

  • Jet Lag Disorder: Temporary sleep disruption after traveling across multiple time zones.

  • Non-24-Hour Sleep-Wake Disorder: The sleep-wake cycle drifts later each day. Most common in people who are totally blind.

  • Irregular Sleep-Wake Rhythm Disorder: No clear sleep-wake pattern; sleep is fragmented throughout the day and night. Often seen in people with dementia or other neurological conditions.

How are they diagnosed?

 

Diagnosis is based on your sleep history, often supported by a sleep diary or a wrist-worn activity monitor (actigraphy) worn for 1–2 weeks.
A sleep study is usually not needed.

How are they treated?

  • Light therapy: Timed exposure to bright light can help shift your internal clock. Morning light helps advance (move earlier) your sleep schedule; evening light helps delay (move later) it.

  • Melatonin: A natural hormone that signals your body it's time to sleep. Taking melatonin at the right time (usually in the early evening for delayed sleep phase) can help reset your clock.

  • Sleep scheduling: Maintaining a consistent sleep-wake schedule, even on weekends, is essential.

  • Workplace adjustments: For shift work disorder, strategic napping, light exposure, and schedule modifications can help.

FAQs

  • Not necessarily. Many people naturally prefer later bedtimes. It becomes a disorder only when the mismatch between your internal clock and your required schedule causes significant problems with sleep, daytime functioning, or distress.

  • There is a biological shift toward later sleep timing during adolescence. This is normal, but when it becomes extreme and interferes with school or daily life, it may be delayed sleep-wake phase disorder.

  • Melatonin can be helpful, especially for circadian rhythm disorders, but timing and dose matter. It works best when taken at the right time relative to your internal clock. Talk to your doctor about the best approach for you.
